Job Description

LMSW - Social Worker

Location: Brooklyn, NY

Schedule: Mon-Fri, 9AM – 5PM

Salary: $61,000-$72,000

Additional Compensation: $1,000 sign-on bonus and $2,000 retention bonus after one year of employment

About the Ro le

We are seeking a dedicated and compassionate Social Worker to join our team. This position plays a critical role in supporting youth and families by collaborating with an interdisciplinary team to assess treatment needs, plan and coordinate services, and provide short-term interventions as necessary. The ideal candidate will uphold our organization’s mission and work to help individuals see their potential.

Key Responsibilities

  • Conduct intake and admissions activities in collaboration with other team members.
  • Initiate discharge planning and build permanent connections for participants.
  • Perform psycho-social evaluations, diagnostic assessments, and coordinate evaluations by other providers.
  • Facilitate treatment team meetings, develop treatment plans, and ensure their monitoring and implementation.
  • Work with the team and placement services to facilitate transitions to long-term care or discharge.
  • Coordinate mental health-related activities and provide crisis intervention services.
  • Provide short-term individual and family therapy where appropriate and support behavioral management interventions.
  • Facilitate psychoeducation groups for participants.
  • Promote permanency through reunification, foster placements, adoption, and alternative living arrangements.
  • Maintain proper record-keeping and documentation in compliance with regulatory agencies.
  • Implement Sanctuary Model tools and practices to create a trauma-sensitive culture.
  • Attend and contribute to internal and external meetings.
  • Perform additional duties as required.

Qualifications

  • Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required.
  • Minimum of two years of experience in child welfare or a related setting.
  • Master’s degree in Social Work required.
  • Experience working with youth and families, including crisis management and intervention.
  • Strong communication, organizational, and computer skills.
  • Ability to work independently and as part of a team.
  • Ability to use public transportation and attend off-site meetings or home visits.
  • New York State Driver’s License preferred.
  • Bilingual in Spanish preferred.
  • Must be available to work flexible hours, including evenings, weekends, and holidays, with possible on-call responsibilities.
  • Commitment to a strength-based, youth development perspective, and trauma-informed care.
